+ $150 one-time enrollment fee
For children ages 10–17. Requires an adult membership.
Monthly fees cover primary care visits. Labs and imaging are separate, offered at cost with no markup. No contracts — cancel anytime.

All membership plans include the same core primary care services. Here's what that looks like in practice.
Preventive care
Annual wellness visits, screenings, and health maintenance. Vaccination recommendations are provided and kept current — vaccines are not administered in the office and must be obtained at a pharmacy or public health clinic.
Chronic disease management
Ongoing care for conditions like diabetes, hypertension, asthma, and thyroid disease.
Acute & urgent care
Same-day or next-day visits for illnesses, infections, injuries, and urgent concerns.
Mental health support
Screening and management of anxiety, depression, and other common mental health conditions within primary care scope.
Care coordination
When you need a specialist or hospital care, I'll help coordinate and make sure nothing falls through the cracks.
